Request Transcript
If you earned a high school equivalency certificate in New Hampshire, you may need a transcript to apply for a job, continue your education, or join the military.
A transcript is proof that you passed the HiSET or GED test.
The paper certificate you got when you passed is just for you and should not be shared.
You may request as many transcripts as you need.
There are two ways to get transcripts:
- If an employer, school, or other organization needs the transcript, they may give you a form to sign that gives them permission to get your transcript from us themselves. They should send the signed form to High School Equivalency Office, 25 Hall Street, Concord, NH 03301 or fax it to (603) 271-3454.
- You can request an official copy of your transcript yourself. Here's how:

Then print and fill out the form. Don’t forget to sign it!
Mail your form
The fee for each transcript is $10.00. It must be paid by check or money order made payable to the State of New Hampshire. Please include payment with your completed Transcript Request form and mail it to:

Do not email the form. Transcripts cannot be requested or sent by email because they contain your personal information.
